This page explains what this website collects, where it goes, and how you can have it deleted. It is written plainly on purpose. If anything here is unclear, email me and I will answer it directly.
I am Henry Owens, MSHS, PA-C, founder and clinical provider of Blackline Performance Health, LLC. The practice is telehealth only and serves Florida residents. There is no clinic address. You can reach me at info@blacklineperformancehealth.com.
If you fill in the form on my contact page, it collects exactly the fields you see on it:
• First name and last name
• Email address
• Phone number
• Age
• The city in Florida you are located in
• A free-text box asking about your goals
That last box is open text. Whatever you type into it is stored word for word, exactly as you wrote it. Because people sometimes describe symptoms or health history there, please treat it as an ordinary email rather than a medical record: send only enough for me to know why you are getting in touch, and save the detail for the consultation itself.
The site also records the page the form was sent from, and the date and time it arrived.
When you submit the form, its contents go to three places:
• To my inbox, as an email notification, so I can reply to you.
• To Webflow, the company that hosts this website, which keeps a copy of the submission in the site's own records.
• To Primara 365 LLC, the marketing company that manages this website for me. Their system receives and stores the submission on my behalf so that an enquiry is not lost if an email fails to deliver.
Primara 365 LLC holds that information only to pass it to me and to confirm the form is working. I do not sell your information, and I do not share it for advertising.
This site uses Google Analytics to understand how it is being used. It sets cookies in your browser and collects:
• which pages you viewed and how long you stayed
• an approximate location, at city level, worked out from your IP address
• your device type, browser and screen size
• the site or search that sent you here
• a record that a contact form was successfully submitted
On that last point, to be precise: Google Analytics is told that a submission happened. It is not sent your name, your email, your phone number, or anything you typed into the goals box.
If you would rather not be measured at all, Google publishes a browser add-on that turns Analytics off everywhere you go: tools.google.com/dlpage/gaoptout. Most browsers also let you block or clear cookies in their settings.
The contact form is a way to get in touch before you are a patient. It is not encrypted messaging, it is not a medical record system, and it is not monitored around the clock. Do not use it for an emergency. If you have a medical emergency, call 911.
Once you become a patient, the records created during your care are handled separately from this website and are kept under the rules that apply to medical records in Florida.
Email info@blacklineperformancehealth.com with the subject line Delete my information, from the email address you used on the form, or telling me which name and email to look for.
I will delete your submission from my inbox, from the website's stored form records, and from Primara 365 LLC's system, and I will confirm to you when it is done. I aim to do this within 30 days. You can also ask me for a copy of what was submitted, and I will send it.
One limit worth stating honestly: if you have already become a patient, medical records created during your care cannot simply be deleted on request, because Florida requires providers to retain them for a set period. That applies to your care records, not to a website enquiry.
This site and this practice are for adults. I do not knowingly collect information from anyone under 18. If a form is submitted by a minor, email me and I will delete it.
If what this site collects changes, I will update this page and change the date at the top. There is no archive of earlier versions.
